The Anatomy of a Headache: Exploring the Upper Neck

When patients struggle with chronic headaches, they often assume they have a bulging or slipped disc in their upper neck. However, there is actually no spinal disc between your skull and your first vertebra (C1, the Atlas), nor is there a disc between C1 and your second vertebra (C2, the Axis).

The Atlas is a ring of bone that carries the weight of your head, while the Axis features a vertical peg that allows your head to pivot when you shake it to say “no”. When these top two bones misalign, they do not pinch a disc—instead, they apply direct, dangerous mechanical pressure to your midbrain and brain stem. Because the midbrain acts as your body’s main neurological relay center, this stress quickly disrupts your balance, coordination, and pain tracking.

How Misalignment Traps Fluid and Toxins in the Skull

Recent scientific developments utilizing motion MRI machines have completely transformed our understanding of upper cervical care. Research conducted by imaging patients during a live chiropractic adjustment demonstrates exactly what occurs when the Atlas or Axis is corrected.

When your neck loses its healthy structural curve due to modern habits like looking down at a smartphone (“text neck”), your spinal cord is pulled downward. Your spinal cord cannot slide up and down freely because it is anchored to the walls of your vertebrae by strong dentate ligaments. As a result, the downward tension pulls the midbrain into the narrow opening at the base of the skull, creating a physical dam.

This structural blockage causes two major issues:

  • Trapped Cerebrospinal Fluid: Your spinal fluid cannot circulate or regenerate normally, trapping toxins inside your cranium and triggering severe, agonizing migraines.
  • Restricted Venous Return: Blood enters your head through the arteries without an issue, but it struggles to exit through the restricted veins. As your arteries pulse with every heartbeat, the backed-up blood presses hard against the veins, causing a throbbing, rhythmic migraine headache.

When a chiropractor adjusts the upper cervical spine, the midbrain moves back up into its proper position inside the cranium. This clears the blockage, allowing cerebrospinal fluid and blood to circulate freely once again. This sudden restoration of fluid dynamics is why some patients feel slightly lightheaded for 10 to 15 seconds right after an adjustment—a sign that the structural pressure causing the headache has finally been cleared.

Cervicogenic Headaches vs. Tension Headaches

Not all headaches originate in the upper cervical fluid pathway. We routinely diagnose and treat other common variants:

  • Cervicogenic Headaches: By clinical definition, these headaches originate from structural misalignments and altered mechanical movement further down the neck.
  • Tension Headaches: These typically present as a tight, agonizing band squeezing all the way around your head, heavily driven by poor daily posture and chronic muscular fatigue.

In rare instances, a patient will report a severe headache that completely vanishes the moment they lie down flat, but returns aggressively when they stand or walk. This symptom pattern points directly to a misalignment in the sacrum or tailbone.

When you walk, your sacrum naturally rotates back and forth, acting as a mechanical pump to push cerebrospinal fluid up your spine into the brain. If the sacrum becomes fixated and locked, the pump stops working. Lying down changes the gravity vector, allowing fluid to pool naturally, but standing up instantly starves the brain of proper fluid circulation.
